T-ACT is a program designed for young adults, ages 18-25 years old, who have serious mental health issues and are homeless, at risk, or otherwise disenfranchised. An assertive outreach approach assists clients with life skills, social skills, housing, jobs, medication management, and other basic needs. The program is collaboratively staffed by St. Louis County Social Services, Brightwater Health, and Life House.
